- Over 17 years of successful Pediatric Day Health Care serving beneficiaries for approximately 1.5 million hours of services to children age 0 –21 years in San Diego County without a single negative outcome.
- Overcame the problem that originally existed wherein medically fragile children were readmitted to an acute care hospital for “social readmission” because the caregivers could no longer cope with caring for the children. This process was expensive and placed potential additional stress on the child. Further, this process had negative social implications for both the child and the entire family.
- Together We Grow serves the 1% of the most medically fragile children in the population. Our program has increased the population of foster parents willing to accept a medically fragile child by providing quality and complete pediatric day care and preschool during the week while they work. It also gives them an occasional weekend of respite to support their efforts.
- In our last two years of children graduating from our preschools into kindergarten, 92% of our graduating class was academically and socially prepared to go to their neighborhood typical kindergarten instead of a special ed program, including our medically fragile children. This gave them an ability to succeed to their highest potential and realize a future where they are more likely to be self-supporting adults.
- Together We Grow is the only Pediatric Day Health Care Center (PDHC) in the state of California that provides overnight respite weekends, an afterschool program, an intersession program for when school is out and an Early Start/ inclusive preschool program that includes typical children as peers to the medically fragile children.
